Palliative Care focuses on the pain, symptoms, and stress of serious illness. Our palliative care services are available to patients in their homes. The initial palliative care house call visit is often provided by one of our nurse practitioners. On occasion, other members of our inter-disciplinary care team may be involved in the course of care, too.
