Janet Chapman, FNP-BC Primary Care Palliative Consultant

Areas of expertise: family primary care, palliative care, patient education, and person-centered care.

Janet began her nursing career in the Kansas City area.  After moving to Arizona, Janet earned her Family Nurse Practitioner (FNP) and Master in   Business Administration (MBA) degrees.

Janet’s experience includes serving as the Executive Director for an Assisted Living Facility.  She has been doing in-home primary care since 2009.

Valerie Bradley

Valerie Bradley, FNP-C Primary Care Palliative Consultant

Areas of expertise: primary care in homes, palliative care, patient education and wound care.

Valerie is a dedicated professional with over 9 years of Family Nurse Practitioner experience in geriatric, palliative and subacute medicine.  Valerie is Wound Care Certified and is currently enrolled in a course to obtain her Palliative Care Certification.   She has a commitment to providing compassionate, holistic care to patients with multiple complex health needs in the comfort of their own homes.

Mary Abrahamson, DNP, APRN, ANP-C Primary Care Palliative Consultant

Areas of expertise: hematology and oncology

Philosophy:  the provision of compassionate whole-person healthcare that promotes quality of life while addressing the unique needs of each patient.

Mary’s experience includes internal medicine, endocrinology, and 15 years of hematology and oncology.  Her passion is to provide patient-centered care that includes collaborative treatment and symptom management as part of spiritual, physical, and emotional health.

Josiah Tooley

Joe Tooley, FNP-BC Primary Care Palliative Consultant

Areas of expertise: family primary care, palliative care, patient education, mental health and person-centered care.

Joe began his career in healthcare in Colorado as a Licensed Practical Nurse.  He continued his education step by step and obtained his Master of Science in Nursing in 2019.  Joe has experience in multiple specialties and loves providing quality, person-centered primary care to those in need.
